ACAT - Aged Care Assessment Teams

Before applying to Donwood, prospective residents will need to be assessed by an Aged Care Assessment Team, to determine the level of care they require. Doctors, community nurses, and social workers can put you in touch with an Aged Care Assessment Team. Applying to Donwood

Anyone is welcome to submit an application however, priority will be given to residents of the City of Maroondah, or applicants whose children or close relatives live in the Municipality and who would benefit from the support of living near a relative.  Please contact Donwood to collect an application for permanent residency.

Interviews covering medical and social details must take place before any decisions are made. Current Assessment Forms must accompany all applications.
